Customer Review Prize Draw Terms & Conditions

FULL TERMS AND CONDITIONS FOR CHECK MY BODY HEALTH LTD CUSTOMER REVIEW PRIZE DRAW

1. Open to any customers who leave a review on the Check My Body Health Trustpilot page aged 17 and over, excluding employees of Check My Body Health Limited, other third parties involved in this promotion and members of their respective households. Persons are deemed to be members of the same household where they share the same postal address or e-mail address.
2.  To Enter: entry into the monthly prize draw is automatic upon submission of your email address where promoted at the start of the Check My Body Health Trustpilot review process (“the Review”). Each monthly prize draw will open at 00:00 on the first Monday of the month, and close at 23:59 on the third Friday of the month (“the Closing Date”).
3. If you enter into the prize draw and subsequently disagree with these Terms and Conditions, or decide that you wish to opt out of the prize draw please contact Check My Body Health Limited at any time before the Closing Date by writing to: Testing Suite 7, The Old Granary, Northampton, NN4 8HP; or email promotions@checkmybodyhealth.com and your entry will be withdrawn.
4. One response to the Review equals one entry. Only one entry per person is permitted into this prize draw.
5. This promotion is only available through completing the Review and providing your email address before the Closing Date.
6. The Prize: One Amazon gift card with a value of £50.
7. Five winners will be selected each month in a random draw conducted on or around the last Friday of every month under the supervision of an independent observer whose decision shall be final (“the Draw”). Each Draw shall contain the total number of entrants to the prize draw for that calendar month.
8. The winner will be contacted by e-mail within 5 working days of the Draw. If the winner does not respond to this notification within 31 days of the notification being made, he/she will be considered to have waived his/her prize, and the prize awarded to a reserve selected at the time of the first selection.
9. Please allow 28 days after the Draw for receipt of prize.
10. Prizes are as stated, subject to availability and there is no cash alternative. All prizes are non-transferrable.
11. The winner undertakes to make no claim in liability against the Promoter with respect to the prize, or its delivery, condition, attributes or any consequence suffered by the possession or use of the prize.
12. The Promoter shall not be liable in the event of any malfunction of the Internet that prevents access to the prize draw or its proper progression.
13. Participation in this prize draw implies awareness and acceptance of the characteristics and limitations of the Internet, lack of protection of certain data against possible misappropriation or pirating, and risks of contamination by any viruses circulating over the system.
14. Participation in the free prize draw implies full acceptance of these Terms and Conditions. Failure to comply with the Terms and Conditions shall entail cancellation of the draw entry.
15. We will only use your personal information in accordance with the Privacy Policy which can be found at https://checkmybodyhealth.com/privacy-policy. In particular, by participating, entrants agree that in the event of being selected as a winner, they may be asked to take part in reasonable publicity related to the promotion, including the publication of their name, initial, or country (if applicable) and any photographs.
16. By participating, the winner agrees that their name and county will be made available to any person who contacts the Promoter and requests details of the winner. To find out the name of the winner, please send an email to promotions@checkmybodyhealth.com.

18. These terms and conditions are governed by the laws of England and Wales and are subject to the exclusive jurisdiction of the English courts.
